Start the process of joining our program

Enrollment:

Age Requirement - Between the ages of 4 weeks and 13 years old.

The Department of Children and Families Office of Licensing, The Department of Health and Senior Services, and Bright Beginners Academy, LLC require information regarding your family and children.
This information is stored on file at the center.

Each file must contain the following:

Registration Fee Custody documents, if applicable Completed and signed registration form Completed and signed papers in a separate packet: Emergency Contact, Disciplinary Procedures, Policy on the Release of Children, Medical Form, Allergy Form, Universal Health Form, Immunization Form, Authorization for Emergency Treatment, Blanket Permission for Walking Trips, Program Agreement, Photo and Video Release Form Annual Physical & Flu Shot Immunization Records (Updated Yearly)
